  1. Rather than using the terms “extension” and “flexion”, why not simply say bend backward and bend forward, respectively. Better to keep things simple. Mnemonics – FADEF = Forward Aggravate Disc Ease Facet; BAFED = Backward Aggravate Facet Ease Disc.

  2. Great explanation, but what if you have both? I have facet joint athritis and foraminal stenosis, but also extruded disks at L3/L4 and L4/L5 compressing the nerve roots. Waiting for nerve root block at L4/L5 but also medial branch blocks for the facet joints. I am in so much pain I’m going out of my mind.

  3. hello dear Dr. I have thoracic spine pain in the T6 region. The pain increases when driving on a bumpy road, riding a bicycle, wearing stiff shoes, carrying a 10 kg load, and similar activities. I had an MRI, but the results were normal. I struggle with this issue every day. I often have to cancel plans to avoid pain.

  4. Excellent video, one tip, instead of flexion and extension talk about bending forward or back because ordinary people usually forget which is flexion in the lower back.

  5. I wish someone had told me that playing sports, especially at a heavy weight, was going to tear up my back. I have facet joint problems with endplates, L3 to S1 degenerative disc, traped nerve, and stenosis. So "the risks with surgery outweigh the benefits." They say they can't fix one issue because they don't believe that it will stop ghe pain. Add the hip damage from sports and t-bone car accident. So much fun😐

He is the only dedicated Dr surgeon that traveled the world went to Vietnam . He studied chronic pain most all his life and specialize pain in the spine. The King of all pain is called Adhesive Arachnoiditis. He explains in his many videos. He believes in opoids for pain patients . Studies shown that taking opioids for many years caused no problems and they were cost effective not like the invasive techniques that caused a lot more harm to the pain patients. He said he took the hypocratic oath and he's not with these anti opioid organizations like CDC FDA and the DEA . They haven't stated any facts to prove that opioids are harmful . Of course there are people that don't go by prescribed amounts they are the ones who probably died from all kinds of different medications they take not reading any labels when to take any medicine .plus the many people who died from street drugs trying to get relief from their high chronic pain . Dr Forest Tenant says heroin is killing them but the reason why they did turn to street drugs not for a high as no chronic pain sufferers wants a high they only want their high constant throbbing burning stabbing pain to go away and because of the war on opoids due to Trump passing a law about perscribing opoids was for his own political gain. The street drugs were killing pain patients forced to the last resort to he to their medical Drs refusing to write them prescription opoids that had killed the severe pain for many years and suddenly cut off. Dr Forest Tenant wrote the Intractable Pain Law and The Pain Bill of Rights in California . He leaves many videos explaining why the Chronic Pain Sufferers needs opoids to kill the pain. Many pain patients have died due to the chronic pain sufferers having a more intense pain because they don't have anything for pain and their blood pressure rose thru the roof 244/100 and higher stroke high BP. The CDC doesn't talk about how not having pain killer opoids to kill the pain and helps them function with quality of life for how many hours the opioid pill lasts until the time is up . The pain never cease and is always there the opoids does kill the pain not the patient . You take all medicine be it blood pressure carefully . The CDC thinks Chronic Pain Sufferers are addicts because they have to be taken the opioid for so many years . Dr Tenant said it's a myth. Chronic pain does not magically go away because the Dr took away the pain patients painkiller opoids. See his many videos . Pass the word about this video to as many entities as possible . This will help lawyers to change this war on Opoids the AMA CDC and DEA has gone too far to have innocent legitimate Chronic Pain patients to Suffer needlessly. If you are a chronic pain sufferer you can join in with a class action lawsuit to sue the CDC etc. Keep watching for the class action suit to begin . It is in the gathering of information about chronic pain and the usage of taking opioids. Winning this case isn't just about money it's for a life back to be able to get painkiller opoids for your lifetime of chronic pain. See Dr Forest Tenants videos. He died at 83 . He left a legacy his videos and his studies about Chronic Pain in California at his humble practice. He would want us to win the fight . It took him years of training MDs to go his way about Chronic Pain.

  7. My compliments to you for pronouncing "facet" properly! You are only one of 2 out of 3-4 dozen doctors and PT's I've listened to on YT that understand that they call it a "fa'-sit" because we're dealing with 2 adjacent surfaces that should normally move across one another in a complimentary fashion. Spread the word to other dipstick doctors who only got "C"'s in English class, that the word has been in existence for hundreds of years before our modern medical anatomists named them!!
    But I am here because I believe arthropathy is what is causing my foraminal stenosis of L4, L5, and S1. Possibly at my L3 facets or vertebral endplates as well. Had 24 non-surgical decompression treatments with little improvement. But a week ago I went back to one of my DC chiropractors who has a lumbar traction/flexion table. Oh my God! The relief is so much more pronounced than with the prior vertical decomp sessions I had! I have MRI's but need a trained doctor or clinician to look whether it could be a facet joint or a vertebral endplate with some moderate spurring going on. The clinical pathology report states such. The evidence you confirmed here says it may be both somewhat; but by going on the flexion table he's achieving a hydrating of the first few discs in a more effective way. I hope it continues to improve; but I've got surgeon's phone numbers waiting if push comes to shove.
    And I would only consider an endoscopic laminotomy or decompression by removing osteophytes with their endoscopic ultrasonic drills now available to spine surgeons. Wish me luck and Godspeed! Thank-you!

  8. I've got 3 minor disc bulges,and facet joint irritation in my lumbar region.L5/S1 for the facet joint problem.But according to my Occupational health 'expert' I shouldn't have been feeling any pain? Probably going to lose my job, because they think I'm not actually injured So frustrating.

  10. What if you have both, disc and facet? Or a myriad of other issues like myself? I have Neurofibromatosis, scoliosis, lordosis, bulging discs, osteoarthritis, facet disease, and spina bifida occulta.

Hello sir in one of my pateint both flexion n extension is painful so no bias seen, radiculpathu ruled out , n its more like LS strain rt more than left…still wigh 3 days of session he is not feeling any better, have also seen SI jointbtest r positive as mentioned….so wt possible test left if u coudl share sum knowledge on this. Thanku
