Lumbar facet syndrome is a common but often misunderstood cause of lower back pain. If you’re experiencing persistent stiffness in your lower back, pain that radiates down the buttocks or thighs, or discomfort that worsens with prolonged sitting or certain movements, you may be dealing with facet joint dysfunction in the lumbar spine.
What is Lumbar Facet Syndrome?
An overview of the lumbar spine and facet joints—small joints that provide stability and allow movement in the spine. When these joints become inflamed or degenerated, it leads to lumbar facet syndrome.
Causes of Lumbar Facet Syndrome:
Aging and Degeneration: Over time, the cartilage in the facet joints can wear down, leading to osteoarthritis and joint dysfunction. We’ll explain how disc degeneration and wear-and-tear on the facet joints contribute to the condition.
Repetitive Stress: Learn how repetitive activities, such as heavy lifting, twisting, or bending, can accelerate the breakdown of the facet joints. We’ll also discuss how poor posture can play a role.
Injury and Trauma: Acute injuries to the lumbar spine, such as falls or motor vehicle accidents, can damage the facet joints and lead to inflammation.
Obesity and Spinal Alignment: Extra weight can increase the stress on the lumbar spine, accelerating the degenerative process and leading to pain.
Symptoms of Lumbar Facet Syndrome:
Understanding the clinical presentation: dull, aching pain localized to the lower back, pain that radiates into the buttocks or thighs, and stiffness that may worsen with prolonged sitting, standing, or certain movements.
How facet joint dysfunction can mimic symptoms of sciatica, and why it’s important to differentiate between the two.
Diagnosis:
An overview of how healthcare providers diagnose lumbar facet syndrome, including a physical examination, diagnostic imaging (X-rays, MRIs), and possibly facet joint injections to confirm the diagnosis.
Treatment Options for Lumbar Facet Syndrome:
Conservative Management: The first line of treatment for facet syndrome often includes physical therapy to strengthen the muscles around the spine and improve posture. We’ll discuss stretching exercises, postural modifications, and activity modifications.
Pharmacologic Treatment: Non-steroidal anti-inflammatory drugs (NSAIDs), muscle relaxants, and other pain management options.
Injections: The role of facet joint injections and epidural steroid injections to reduce inflammation and provide pain relief.
Preventive Measures and Lifestyle Modifications:
Learn practical tips to prevent exacerbation of facet joint pain, such as maintaining a healthy weight, improving posture, and avoiding repetitive strain on the lower back.
At-Home Exercises for Pain Relief:
We’ll show specific stretches and strengthening exercises to alleviate symptoms and improve spinal mobility. These exercises target the lower back and surrounding muscles to provide stability and reduce the load on the facet joints.

Very well explained Thank you so much. It was helpful.
Just diagnosed with it. 😢
Best video I've seen on this subject, with information on the condition as well as stretches and leveled strength training. Thank you!
Thanks
I have severe facet joint syndrome and on pain contract. I'm 56 on hydrocodone, soma once day muscle relaxer and ibuprofen and nothing helps. I've done PT,ice and heat therapy, injections. My doctor forcing me go Pain clinic to take my hydrocodone away and put me on Buprophenene which is for addicts OUD. I've never failed drug test. I'm being set up for failure. It's unethical to let a chronic pain patient suffer.
I have watched many video's and this is the one I follow..although, by the time I finish the exercise, my hamstrings/glutes are burning and my feet goes numb so I'm not sure if I chose the correct video for my condition. Still, thank you for this video and your thorough explanation.
What do you do when it’s effecting your Baxter nerve???
I got instant relief after doing the stretching and exercises. My only problem is I'm having difficulty doing the side planks of 8-12 reps in 2 sets. I can only do 2 x 30 secs reps for each side 😀
im having trouble figuring out if my lumbar pain id facet related, or disc related. i have DDD in l4-l5 and l5-s1, retrolisthesis in the same vertebra and facet joint hypertrophy. are these exercises safe for any of these conditions?
My neurologist thinks I have this. I have motor weakness in both legs, it all started 15 years ago after I went to help my child off the floor. I was dismissed as just having a burst disc – it never healed. My nerves are angry and did not like physiotherapy.
Odd, this video has no sound. I can only hear static where it should be.
Really some good information. I’ve already had 3 back surgeries fusion L5 to S1. I was fine riding road bike average 100 miles a week. Then found out I had heart condition. I sat for about 4 years after 2 stents in heart. I lost all muscle legs glutes core. I do have narrowing of L 2 3 4. Right now all pain at L5 to S1 wrapping around lower back. You explained exactly what I have going on now. I’m 72 but been active in some kind of sports. Really a good video. I got a lot from it TY
Cure for back pain forever
A simple xray diagnosed this for me. I have mild facet arthritis in L4-L5 arra and L5-S1. It sure can get painful for being mild. I was going to go to physical therapy. But…im going to try this first. Thank you.
That standing ball stretch is better than sex
I've had low back pain for many years but it got bad enough to go to a doctor about 2 years ago. I've been diagnosed with several things but generally spinal stenosis and facet joint issues. I had RFA last year but I can't say it helped much. I still find it hard to stand or walk for extended periods. I thought I possibly also had SI pain on my left side due to my desk job but I've also been told that can be originating elsewhere. That's the pain that sent me to the doctor. Can that also be facet-related? PT had me bending backwards and that was painful so now I concentrate on bending forward. It's been a real journey trying to pinpoint the exact issue and how to fix it.
What about for golfers?
Right now, I have a sore back big time, seven days, from hitting a golf ball slightly different way. I’m trying to alter my swing.
Does any of this relate to what happened to me?
Probably the best video on the topic. Well done Sir, thank you!!
Good Luck trying to pinpoint where all your pain is coming from . Even the most qualified doctors will just scratch their head 🤣
Thank you so much.i have this problem,i feels bad while walking, laying was better, sitting i feel very bad and pain in the tailbone.how do you suggest?please😊
With me , I have a pain in the equana Area , When I stand or move around it becomes Extremely painful . The more I move around the more Intense the pain and the Longer it takes for the Pain to subside. I have had x-rays/MRI/CT scans which do show I do have some facet joint damage … But I can't get a reasonable Diagnoses or what to exactly tell my Docs …
sir, this was amazingly explained. thank you so much. God bless you 🤩. I was doing Aerial yoga & I was pushed to do an extended back bending where now I have facets issue in last few bones of spine. yoga stopped since then its been 2 weeks now. I need your advise, lying down relives the pain, standing increases the pain but sitting for office work more than 1hour makes the pain worse. is bed rest suggested if yes for how long and can we do walking if yes how much time are we allowed simple slow walking per day. does vitamin & calcium supplements help in this case ? will i ever be able to start Yoga again?
